My husband insists on taking off his clothes too. He also says he sees things on the floor. I told him he had delusions before when he had a fever. So I gave him two Advil and told him once the fever breaks the delusions will go away. I’m waiting to see if it works.

They have given him medication. He is on 25 mg of Quetiapine, which is a very small dose, but he has Parkinson's, and they say it could cause a stroke or even death, and he is also on 50 mg of Trazodone and the generic for Zoloft (not sure of mg), but it hasn't help his agitation or aggression.

I posted earlier that I gave my husband two Advil and told him his delusions would go away once his fever was gone. As crazy as it sounds, it worked to an extent. He stopped seeing things on the floor (like safety pins and tiny springs). But he still pulls his clothes off. I stopped trying to keep pajama bottoms on him. He wears a tshirt, a long sleeve shirt, socks, and a diaper.
Now when he claims to see things I ask him if he is too warm and check him for fever. He tells me if he thinks he needs an Advil.
He has just started asking to call his mother or father. They both passed away many years ago. I gently tell him he cannot call them because they are gone. He remembers and is sad for just a moment then moves on.
The destruction of his mind and personality is more difficult for me than his physical problems.

Thanks for your answer. At least it's nice to know he's not the only one. Can't figure out why he does it. Sometimes, I think he's just too warm. My husband had a few hallucinations seeing people in the hallways at first, but they seem to have gone away with some of his medication.

What does his doctor say? Are there meds that can help?
