At What Stage Is It Appropriate To Begin Talking To My Neurologist About Late-stage Parkinson's?

I am finding that my symptoms are progressing slowly but nevertheless progressing. I'm curious if it is a good idea to begin the progression talk with my neurologist now. If not now, how far does my Parkinson's progression have to get before beginning the talk about late-stage care?
